Therapeutic exercise associated with transcutaneous electrical nerve stimulation reduces pain and increases functionality in knee osteoarthritis individuals

Objective: To compare the effect of Transcutaneous Electrical Nerve Stimulation (TENS), therapeutic exercise and the association of both techniques on pain, walking time, muscle strength and flexibility, as well as the functionality of individuals with knee osteoarthritis (OA). Materials and Methods: The research is characterized as a randomized, prospective and blind clinical trial. Thirty-six patients participated, of which 30 completed the treatment, and were divided into three groups. Group 1 submitted to TENS; group 2 to the therapeutic exercise; and group 3 to the combination of the two techniques (TENS and therapeutic exercise). A total of 10 sessions in each group, three times a week, were conducted. The visual analogue pain scale (EVA), the 8-metres walking time, the one repetition maximum tests (1 RM), as measured by the posterior chain flexibility using the Bank of Wells and the Lequesne Algofunctional Questionnaire were evaluated. Results: All groups presented significant pain reduction (p<0.0001 in all groups) and functional improvement (p=0.0018 in G1, p=0.0003 in G2, and p=0.0078 in G3). The groups treated with therapeutic exercises and with the association of techniques increased muscle strength and flexibility. Conclusion: The use of TENS may help reduce pain and function in knee OA patients. Moreover, the performance of therapeutic exercises associated or not with TENS reduced the pain and increased the functionality, as well as incremented quadriceps muscle strength.
