Abstract

Objective: To analyze the association between history of falls, concern about falling, suggestive signs of sarcopenia, and mean knee muscle power in community-dwelling older adults, comparing fallers and non-fallers.

Methods: This was a cross-sectional, observational study conducted with older adults recruited through online dissemination and among participants of the older adults’ community center of the Universidade Católica de Brasília (UCB), with assessments carried out at the Physical Assessment Laboratory (LAFIT-UCB), in Taguatinga Sul, Federal District, between January and December 2024. The sample was divided into fallers and non-fallers based on the number of falls in the previous 12 months. The Falls Efficacy Scale International (FES-I), the Simple Questionnaire to Rapidly Diagnose Sarcopenia (SARC-CALF), and the Biodex System 3 isokinetic dynamometer were used. Data were analyzed using the Shapiro-Wilk test, Fisher’s exact test, Mann-Whitney U test, and GraphPad 8.0 software.

Results: There was no significant difference in concern about falling (FES-I) between the groups. However, the proportion of participants with extreme concern about falling was higher in the faller group (21%) than in the non-faller group (9%). In the odds ratio analysis, no significant association was found between the occurrence of falls and high concern about falling. Regarding SARC-CALF, there was a significant difference between the groups, with suggestive signs of sarcopenia being more frequent among fallers (27.7%) than among non-fallers (5.7%). In the isokinetic assessment, mean dominant knee extension power was significantly lower in the faller group [67.4 (IQR: 47.6)] compared with the non-faller group [89.3 (IQR: 58.6); p=0.033].

Conclusion: This study demonstrates that older adult fallers present greater concern about falling, suggestive signs of sarcopenia, and lower mean muscle power of the knee extensors compared with non-fallers.
