Estudo da biópsia por agulha cortante no diagnóstico histopatológico de lesões bucais
Abstract
Objetivo: Estudar a eficácia da biópsia por agulha cortante (BAC) no diagnóstico de lesões bucais localizadas em tecidos moles, indicadas para biópsia incisional ou excisional, em 21 pacientes, de ambos os sexos, com idades entre 18 e 80 anos. Materiais e métodos: Para a realização das biópsias foi utilizada uma agulha cortante de 18 Ga × 9 cm. Cada peça foi submetida a três disparos com a agulha obtendo-se três espécimes que após o processamento laboratorial, originaram três lâminas. Para as análises histopatológicas elaborou-se um questionário contendo duas perguntas sobre o diagnóstico dos espécimes que foi aplicado em dois momentos diferentes, com e sem a história clínica. As respostas do questionário, em ambas as etapas, foram comparadas com os diagnósticos conclusivos das peças cirúrgicas removidas anteriormente. Resultados: Os resultados foram elaborados conforme uma escala de valores e submetidos à análise estatística de McNemar, a um nível de significância de 5%. O número de acertos dos diagnósticos específicos obtidos com os espécimes da agulha cortante foi estatisticamente superior em relação ao número de erros. Observou-se também que o número de acertos foi maior quando o examinador respondeu ao questionário com o conhecimento da história clínica. Conclusão: A biópsia por agulha cortante mostrou-se eficaz na elaboração do diagnóstico histopatológico de lesões bucais.

SUMMARY
Objective: The proposal of this research work aimed to accomplish a study on the efficiency of cutting needle biopsy (CNB) in oral lesions diagnosis, found in soft tissues, with indication of incisional or excisional biopsy, in twenty-one patients, of both sexes, ranging from 18 to 80 years old. Materials and method: For the accomplish of the cutting needle biopsy, a specific needle of 18 Ga × 9 cm caliber was used. Each specimen was submitted to three shots with the cutting needle, getting three specimens that after the laboratorial processing originated three red-faced. For the histopathological analysis being so, a questionnaire was elaborated for the present work, with two different questions about the specimen diagnosis that was applied in two different moments, without and not clinical history. The results of the questionnaire, in both stages, were compared with the final histopathological diagnosis of the whole surgical specimens initially removed. Results: The results were elaborated according to a value scale and submitted to the statistical analysis of McNemar to a level of significance of 5%.The number of correct answers of the specific diagnosis obtained with the cutting needle specimen was statistically higher in relation to the number of mistakes. It was also noticed that the number of correct answers got to be higher when the examinator answered the questionnaire with the knowledge of the clinical history. Conclusion: It was concluded that, the cutting needle biopsy (CNB) showed efficiency in elaborating histopathological analysis of oral lesions.
