Inspiratory muscle endurance in adults with asthma: a systematic review

Introduction: Asthma is a chronic disease that can lead to impaired inspiratory muscle endurance (IME). Objective: To evaluate the IME profile in asthmatic patients. Materials and Methods: This is a systematic review conducted through the Pubmed/Medline, Lilacs, Scielo and PEDro. We selected studies that evaluated IME in adults (≥18 years old) diagnosed with asthma. The following search strategies were used: 1) Asthma AND Respiratory Muscle AND Physical Endurance; 2) Asthma AND Respiratory Muscle AND Muscle Endurance; 3) Asthma AND Respiratory Muscle AND Muscle Resistance; 4) Asthma AND Inspiratory Muscle Endurance. The methodological quality was evaluated through the adapted Grading of Recommendations Assessment, Development, and Evaluation. Results: In a total of 904 articles, only 04 were included. The studies totaled 137 individuals, including asthmatics dependent/nondependent on corticoids (n=121), as well as control subjects (n=16). For the IME evaluation, the most frequent equipment was the use of inspiratory plungers (3/4), but half of the studies used the incremental load protocol (2/4) and the rest the permanent load test (2/4). All of them showed IME reduction in adult asthmatics, both by the comparison with a control group and by using cutoff points. Conclusion: Asthma causes deleterious effects on IME, leading to an inspiratory muscle capacity reduction in tolerating a particular ventilatory demand.
